| Description: | Histologic sections of skin show a central follicular structure with a dilated cavity opening to the surface. The epithelial lining is thickened, with small projections emanating into the surrounding dermis. A small cyst is present within the lining. The cavity is filled with loose keratin. | |||||
| Comments: | This lesion was removed from the forehead of a middle aged woman, a typical presentation. The histology in this case is also typical. The differential diagnosis would include a pilar sheath acanthoma, trichofolliculoma, and a large comedone. The dilated pore of Winer has more open of a cavity and more clear of a connection with the epidermis than the pilar sheath acanthoma, although these entities have overlapping features. The trichofolliculoma is usually more complex, with secondary and often tertiary follicles arising from the central cavity. A large comedone would not have the epithelial projections shown in this case.
