Description: There is an abrupt step-off of the stratum corneum, forming a central depression. The cornified layer is decreased without much alteration in the underlying epidermis or dermis.
Comments: Circumscribed palmar hypokeratosis is a fairly rare and recently described entity. Patients often present with one or several well-demarcated depressions in their palms and/or soles, as in this case. These are asymptomatic and there is currently no effective treatment. The pathology is distinctive in its abrupt "drop-off" in the stratum corneum. Other characteristics described are mild diminution in the granular layer and perhaps keratinocytic dysmaturation. A defect in keratinization is presumed but a definitive ultrastructural defect has not been identified to date.

Description: Higher power of the same lesion demonstrates the presence of numerous smooth muscle bundles, vessels, and nerves characteristic of nipple anatomy.
Comments: This patient presented with a papule on his trunk, present since birth. Removal was largely cosmetic. Polythelia (supernumerary nipples) may occur anywhere along the "milk line". They are usually asymptomatic and non-functional, but may be submitted as a seborrheic keratosis or to rule out malignancy.

Description: Histologic sections of skin from a different patient show similar features. There is mild papillomatosis with numerous vellus follicles. In the subjacent tissue, there is fibroadipose tissue and central cartilage.
Comments: The accessory tragus is thought to arise from a embryological defect involving the branchial arch. It is often present in a pre-auricular location, but may appear anywhere along the path to the corner of the mouth, or more rarely, over the sternocleidomastoid muscle, as in this case. When in the latter location, some authors use the unflattering term "wattle". This lesion may be confused with a fibroepithelial polyp (acrochordon) or soft fibroma, but the anatomic location and the presence of vellus follicles and cartilage leads to the correct designation.
