Although this dermatosis is not specific of diabetes, it is probably the most common finding in the skin of diabetics. These pigmented pretibial patches may be a marker for underlying cutaneous microangiopathy and similar lesions in the vessels of other tissues and organs..

Description: Histologic sections of skin show a shallow cup-like invagination of the mildly acanthotic epidermis. The cup is filled with keratin and degenerated basophilic material. On the left side of the field, in the dermis, there is an area of degenerated dermis with inflammatory debris, corresponding to a follicle in a different plane of section.
Comments: This 66-year-old African-American woman developed numerous pruritic papules and plaques on her lower extremities. She has diabetes and chronic renal failure, on dialysis. Perforating disorder secondary to chronic renal disease and/or diabetes mellitus is a fairly recently described entity. The acquired perforating diseases overlap considerably (Kyrle, perforating folliculitis), and may represent the same or similar processes. This would be a typical presentation.
