DermAtlas: Histology - melanoma, metastatic
© 2001-2009, DermAtlas
Image Name: melanoma_mucinous_1_070501   File Type: jpg
Diagnosis: MELANOMA, METASTATIC   Category: neoplasm, melanocytic /
neoplasm, metastatic
Body Site: back   Age: 56 years
Contributor: Greg Hosler, MD, PhD
Description: There is a large, well-demarcated tumor within the deep dermis. The cells are small and basaloid, with a mucinous stroma.
Comments: This is an unusual morphologic variant of melanoma. It has both small-cell and mucinous features, showing how melanoma can truly take on any appearance. This morphology is usually associated with elderly patients. This patient is a little young. She had an acral melanoma two years prior. This was a metastasis to the back and only loosely resembled the invasive component of the original. It was S-100 and MART-1 positive (not shown).

DermAtlas: Histology - metastases, salivary gland carcinoma
© 2001-2009, DermAtlas
Image Name: metastatic_salivaryCA_1_070501   File Type: jpg
Diagnosis: METASTASES, SALIVARY GLAND CARCINOMA /
ADENOCARCINOMA, METASTATIC
  Category: neoplasm, metastatic
Body Site: neck   Age: 64 years
Contributor: Greg Hosler, MD, PhD
Description: Within the dermis there are atypical eosinophilic nests admixed with hemorrhage. The epidermis is uninvolved.
Comments: This patient presented with a skin mass. Following the diagnosis of metastatic adenocarcinoma (and an immunoprofile suggestive of primary salivary origin), a mass was found in the parotid gland. Metastatic carcinoma from the salivary gland is unusual. When it occurs, like most metastases, there is a predilection for the skin in the region of the primary tumor.

DermAtlas: Histology - metastases, breast carcinoma
© 2001-2009, DermAtlas
Image Name: metastatic_breastCA_1_070501   File Type: jpg
Diagnosis: METASTASES, BREAST CARCINOMA /
ADENOCARCINOMA, METASTATIC
  Category: neoplasm, metastatic
Body Site: chest   Age: 51 years
Contributor: Greg Hosler, MD, PhD
Description: There is a cellular infiltrate within the dermis. It forms nodules and cords, infiltrating between thickened collagen bundles. The overlying epidermis is uninvolved.
Comments: Metastatic breast carcinoma is the most common metastatic tumor to the skin. Like other metastases, there is a predilection for skin overlying the tumor, as in this case. This particular case is lobular cancer and can be very subtle. Although this biopsy shows a cellular infiltrate, often times lobular cancer will be sparse and missed by the pathologist, and sometimes requires a cytokeratin immunostain for verification.

Diagnosis: MELANOMA, METASTATIC   Category: neoplasm, melanocytic /
neoplasm, metastatic
Body Site: back   Age: 56 years
Contributor: Greg Hosler, MD, PhD
Description: The higher power shows the striking mucinous component to the lesion, resembling a basal cell carcinoma.
Comments: This is an unusual morphologic variant of melanoma. It has both small-cell and mucinous features, showing how melanoma can truly take on any appearance. This morphology is usually associated with elderly patients. This patient is a little young. She had an acral melanoma two years prior. Image Name: metastatic_salivaryCA_2_070501   File Type: jpg
Diagnosis: METASTASES, SALIVARY GLAND CARCINOMA /
ADENOCARCINOMA, METASTATIC
  Category: neoplasm, metastatic
Body Site: neck   Age: 64 years
Contributor: Greg Hosler, MD, PhD
Description: The atypical cells are large and eosinophilic (oncocytic). The cells are polygonal and form ducts. There are pink secretions within the ducts. In several foci, they are plugging lymphatics (not shown).

Diagnosis: METASTASES, BREAST CARCINOMA /
ADENOCARCINOMA, METASTATIC
  Category: neoplasm, metastatic
Body Site: chest   Age: 51 years
Contributor: Greg Hosler, MD, PhD
Description: The individual tumor cells are fairly cytologically bland. They infiltrate as single cells and cords between thickened collagen bundles.

DermAtlas: Histology - metastases, ovarian serous carcinoma
© 2001-2009, DermAtlas
Image Name: metastatic_ovarianCA_1_070501   File Type: jpg
Diagnosis: METASTASES, OVARIAN SEROUS CARCINOMA   Category: neoplasm, metastatic
Body Site: abdomen   Age: 51 years
Contributor: Greg Hosler, MD, PhD
Description: There is a basophilic nodular tumor filling the papillary dermis. There are papillary fronds within each nodule. The epidermis is uninvolved.
Comments: This 51-year-old woman presented with a red papule on her lower abdomen. The histology showed papillary serous carcinoma, consistent with a metastasis from her known ovarian cancer. Metastatic ovarian carcinoma to the skin is uncommon, considerably less frequent than breast primaries.

Image Name: metastatic_ovarianCA_2_070501   File Type: jpg
Diagnosis: METASTASES, OVARIAN SEROUS CARCINOMA   Category: neoplasm, metastatic
Body Site: abdomen   Age: 51 years
Contributor: Greg Hosler, MD, PhD
Description: The cells show marked atypia with pleomorphism and angry nuclei. Small micropapillae form within the nodules with shedding of individual tumor cells. Mitoses are readily identified.
