Description: clustered discrete and confluent 2 mm vesicles and crusts on a red edematous 2 cm plaque
Comments: This 64- year-old man suffered from recurrent Herpes simplex labialis for 10 years. Vesicles over a red plaque accompanied by burning and itching developed on the right cheek 5 days ago. The Quick Tzanck test of material obained from the vesicles revealed balloon cells with thick nuclear and cytoplasmic membranes fusing to form multinucleate giant cells. The eruption cleared in 10 days with topical antibiotic ointment, topical steroid, and 5 days of oral valacyclovir. The Quick Tzanck Test, which incorporates a modified Giemas solution, was devised by Dr. Sino as a rapid screen for the diagnosis of Herpes simplex virus infection. Staining for cytology takes only 2 minutes.

Comments: This 68-year-old man was evaluated for a vesicular eruption on his forehead, eyelids, nose and mandible accompanied by erythema and swelling. The Quick Tzanck Test from the vesicles revealed the infiltration of numerous neutrophils, balloon cells and giant cells. Oral valacyclovir for 7 days, topical steroid over the itching areas, and antibiotic ointment for the scratched wounds were prescribed. Two days later the swelling subsided and the vesicles decreased in size. The lesions resolved completely within 10 days. Eighteen months later the patient exeprienced a recurrence. The positive Tzanck preparation helped his clinicians differentiate this eruption from bacterial infection, contact dermatitis, and drug reaction. He was on medications for hypertension and gout.

Comments: This healthy 13-year-old girl developed a generalized minimally itchy bullous eruption with the greatest concentration of lesions on the trunk and sparing of the face a month ago. Many of the lesions had central crusts and erosions and peripheral flaccid bullae. A skin biopsy revealed subcorneal vesicles and pustules, and direct and indirect immunofluorescence were negative. Two weeks later many of the larger bullae had resolved leaving discrete and confluent hyperpigmented macules and patches with darker edges and new 2-3 mm vesicles and pustules.
