Description: There is a single flesh-colored-to-red papule on the chin. Blanching can be elicitied. It is asymptomatic.
Comments: This young man presented with a solitary flesh-colored to red papule on the chin. The differential diagnosis was a Spitz nevus versus a vascular neoplasm. This case has classic histologic features of a pyogenic granuloma, or lobular capillary hemangioma. Pyogenic granulomas may undergo different stages of evolution. Depending on when biopsied, they may be ulcerated, markedly inflamed, or even sclerotic/hyalinized.

Description: Thre is a vascular neoplasm with a lobular architecture. Each lobule is made up of small-caliber vessels, searated by fibrous septae. The epidermis forms a collarette.
Comments: This young man presented with a solitary flesh-colored to red papule on the chin. The differential diagnosis was a Spitz nevus versus a vascular neoplasm. This case has classic histologic features of a pyogenic granuloma, or lobular capillary hemangioma. Pyogenic granulomas may undergo different stages of evolution. Depending on when biopsied, they may be ulcerated, markedly inflamed, or even sclerotic/hyalinized.

Description: A higher power view shows the small-caliber vessels. Endothelial cells have a reactive look, but significant cytologic atypia is not appreciated. A significant inflammatory infiltrate is not seen.

Comments: This healthy 5-year-old boy with a small portwine stain on the side of his right cheek extending onto the ear developed a red papule within the patch on the cheek 3 weeks ago. The papule was treated successfully with a Nd:YAG 1064nm long pulsed laser using a 7 mm spot, 120 joules/cm2 energy fluence, 20 msec pulse duration, for a total of 2 pulsed.
