DermAtlas: FACE - molluscum contagiosum
© 2001-2009, DermAtlas
Image Name: Molluscum_1_090617   File Type: jpg
Diagnosis: MOLLUSCUM CONTAGIOSUM /
HUMAN IMMUNODEFICIENCY VIRUS INFECTION
  Category: infections and infestations /
immunodeficiency related
Body Site: face / neck
nose / cheek
  Age: 36 years
Contributor: Vlada Groysman, MD
Description: multiple clustered pearly skin colored 2-5 mm papules many with a central; scaly dimple
Comments: A 34-year-old man complained of multiple flesh-colored papues on his face and arms. Although not symptomatic he was worried, because they continued to spread. He was recently diagnosed with Human immunodeficiency virus infection and had a CD4 count of 156.

DermAtlas: LEG - lymphomatoid papulosis
© 2001-2009, DermAtlas
Image Name: Lymphomatoid_Papulosis_HIV_1_080114   File Type: jpg
Diagnosis: LYMPHOMATOID PAPULOSIS   Category: neoplasm, leukemia/lymphoma /
immunodeficiency related
Body Site: leg / thigh / thigh   Age: 53 years
Contributor: Azzam Bittar, MD
Description: generalized symmetric red indurated papules, plaques, and nodules with overlying scale, ulceration, crusting and hypo and hyperpigmentation
Comments: A 53-year-old Human Immunodeficiency virus positive man with a CD4 count of 97 on antiretroviral therapy was evaluated for pruritic red plaques on the extremities, trunk, and right ear. A skin biopsy showed a dense nodular infiltrate, of large atypical lymphocytes, smaller lymphocytes, and some eosinophiles and neutrophils consistent with lymphomatois papulosis. Immunohistochemical studies performed to further evaluate the immunophenotype of the lymphocytes showed that the infiltrate was comprised of CD30 T cells with a few CD20, CD8, and CD7 positive cells.

GRAFT VERSUS HOST DISEASE
Category: cutaneous sign of systemic disease /
atrophy /
immunodeficiency related
Body Site: foot / total body
nail, foot
  Age: 62 years
Contributor: Bernard Cohen, MD
Description: generalized poikiloderma with scale, atrophy, reticulated blanching erythema, and mottled hyperpigmentation, nail atrophy, scale, yellowing
Comments: This 62-year-old man, who received a bone marrow transplant following chemotherapy for leukemia, developed chronic poikiloderma and lichenoid dermatitis thought to be consistent with chronic graft versus host disease. Interestingly, when lymphocyte markers were checked, there were increased numbers of recipient lymphocytes noted in the inflammatory infiltrate on skin biopsy when the skin disease worsened when the dose of immunosuppressives was reduced. This suggested that the disease was related to host lymphocyte injury of altered host antigens possibly induced by immunosupprressive medications.
