Description: large segmental purple plaque with massive central ulceration
Comments: This healthy 10 week old girl was noted to have a red patch on her arm shortly after birth. The patch grew quickly in volume and a large portion of the expanding plaque became necrotic and ulcerated at 4 weeks of age. She was initially treated with 2 mg/kg/day of oral prednisolone without improvement. The ulcer was painful, and several episodes of secondary infection with methicillin staphylococcus were treated with oral and parenteral antibiotics. At 8 weeks of age the prednisolone was tapered, propranolol 2 mg/kg/day was initiated, and the wound was treated with gentle cleaning and a dressing composed of carboxymethylcellulose and 1.2% ionic silver. The first images dated 10/22 were taken 36 hours after the initiation of propranolol and methylcellulose dressings. Subsequent images taken during dressing changes on 10/25 and 10/28 demonstrate a clean granulating wound base.

Description: multiple disseminated golden brown leathery .05-2.5 cm papules and plaques
Comments: This healthy 2-year-old boy developed multiuple golden brown papules and plaques on his trunk and extremities. A dramatic Darier sign was readily elicited by rubbing. Lesions first began to appear at 3 months of age and peaked by 18 months of age. Although the eruption remained stable until age 6, many lesions cleared over the subsequent year. He continued to grow and develop normally.

Description: tender 1 cm nodule with central white chalky papule
Comments: A 60-year-old woman was referred for a painful lesion of the tip of her right thumb. Radiographs indicated diffuse sclerotic lesions in the soft tissues of the distal phalanx of her thumb. She reported an over 15-year-history of Raynaud phenomenon and positive antinuclear antibody titer. The lesion was surgically debrided under local anaesthesia but the borders could not be identified. Biopsy showed localized calcinosis cutis. The patient was referred to rheumatology where the diagnosis of Crest syndrome was made. Two weeks later the sutures were removed, and the scar was ruptured because of residual calcinosis in the soft tissues. By that time the borders of the lesions were easily identified and removed. No residual clinical lesion was evident 2 months post operatively despite radiographic evidence of persistent calcinosis.
