Description: violaceous nodules with overlying scale and crust
Comments: This 8-year-old boy developed 1-2 cm violaceous nodules with overlying scaling and crust most prominent in the largest, primary lesion. These progressed in a sporotrichoid pattern along his chin. A biopsy showed granulomatous inflammation with cultures positive for Mycobacterium marinum.

Description: There is an acanthotic epidermis with focal ulceration. In the dermis, there is a mixed inflammatory infiltrate, extending deep.
Comments: This patient gave a typical history of developing a lesion following a fishing trip. M. marinum may present as a solitary lesion but has been known to have sporotrichoid (lymphatic) spread. In this case, the organsims are not plentiful but readily found on an acid fast stain.

Description: An acid fast stain (AFB-Kinyoun in this case) highlights the mycobacteria in red. Few organisms are found. Several bacillary forms are present in the lower center of the field.

After 3 months of oral trimethoprin/sulfamethoxazole the lesions improved with a decrease in erythema and resolution of the erosions and crusts

Comments: This healthy 45-year-old man developed asymptomatic nodules on his right hand which progressed up the forearm over 2 months. A biopsy showed granulomatous inflammation and cultures grew Mycobacterium marinum. He spent weekends sailing in the Chesapeake Bay.
