| Description: | swelling of the right middle finger and thumb | ||||
| Comments: | A 12-year-old girl developed swelling and restriction of motion of the right middle finger 6 months ago and the left wrist a month ago. She was afebrile and laboratory findings were normal with the exception of the antinuclear antibody titer that was positive at 1:80. MRI indicated inflammatory joint fluid in the left wrist and carpometacarpal and metacarpophalangeal joints of the thumb and on the right proximal interphalangeal joint of the middle finger and the interphalangeal joint of the thumb. | ||||

| Diagnosis: | RHEUMATOID NEUTROPHILIC DERMATOSIS / RHEUMATOID ARTHRITIS | Category: | cutaneous sign of systemic disease / vesiculobullous eruptions / histologic finding | ||
| Body Site: | forearm | Age: | 49 years | ||
| Contributor: | Greg Hosler, MD, PhD | ||||
| Description: | The infiltrate consists almost exclusively of neutrophils. There is no evidence of vasculitis. | ||||
| Comments: | This is a great example of a rare case of rheumatoid neutrophilic dermatosis. This patient had severe rheumatoid arthritis and developed plaques and bulla over the forearms. Rheumatoid neutrophilic dermatosis is a rare manifestation of severe joint disease, as in this case. Histologically, this is one of the neutrophilic dermatoses along with Sweet syndrome, cellulitis, pyoderma gangrenosum and others. The bulla are often secondary, and not always seen. Distinction between these entities is virtually impossible without clinical history. | ||||

| Diagnosis: | RHEUMATOID ARTHRITIS / TORTICOLLIS | Category: | collagen vascular disease | ||
| Body Site: | neck | Age: | 3 years | ||
| Contributor: | Nikolaos Sferopoulos, MD | ||||
| Description: | torticollis | ||||
| Comments: | A 3-year-old boy rwas evaluated for acquired torticollis and a diffuse swelling of the third right toe. Stiffness and limitation of motion but no tenderness of the cervical spine had been noted approximately 6 months ago. A fusiform, painless swelling was also noted on the left third toe. Magnetic resonance imaging(MRI) of the cervical spine indicated no abnormal findings, while an MRI of the foot was indicative of fibromatosis. A biopsy was performed from the toe that showed mild synovitis of the tendons. Knee joint involvement followed 6 months later. Medical treatment for rheumatoid arthritis was then started and the toe swelling rapidly subsided. | ||||

| Comments: | A 2-year-old girl was seen for swelling and restriction of motion of the middle finger of the left hand which appeared 4 months ago. The child was afebrile and had no evidence of systemic disease. Radiographs indicated periosteal reaction of the middle phalanx. Magmetic resonance imaging showed tenosynovitis of the flexor tendons as well as thickening of the synovium with inflammatory joint fluid in both the metacarpal-phalangeal and proximal interphalangeal joints. The antinuclear antibody titer was positive at 1:320. Knee joint involvement followed one month later. | ||||
