Comments: This healthy 2-month-old boy developed widespread scattered crusted papules on a red base some with central erosions. He was healthy and growing well without evidence of systemic involvement. Laboratory studies including liver function tests, electrolytes, complete blood count, abdominal ultrasound, and skeletal survey was normal.

Description: disseminated 2-4 mm red papules many with central crusts and pustules and healed papules with erythema and a collarette of scale
Comments: This 1-week-old boy was born to a 29 year old G2 P0 mother. The mother had a history of Gonnorhea which was treated prior to her pregnancy. The prenatal course was complicated by gestational diabetes requiring treatment with oral medications. Prenatal labs were as follows: Blood type O positive, serologic test for syphilis non-reactive, gonorrhea and Chlamydia cultures negative, rubella immune, and Group-B streptococcus positive treated with ampicillin. There was no history of illicit drug, alcohol, or tobacco use. The delivery was uncomplicated and the child was born at term weighing 2671 grams, with Apgar score of 8 and 9. At birth, baby was noted to have erythematous nodules and crusted papules diffusely scattered over the skin including palms and soles, and oral mucous membranes. Histologic sections of skin show numerous large mononuclear cells containing reniform nuclei with prominent nucleoli and abundant pink cytoplasm infiltrating the epidermis. The cells were also seen infiltrating the dermis in a perivascular and periadnexal pattern. The area of epidermal infiltration was also spongiotic and contained hemorrhage and overlying seroinflammatory crust. A few admixed lymphocytes were also seen in the dermis. The adjacent epidermis was otherwise unremarkable. Immunohistochemical stains demonstrated that the large mononuclear cells were reactive with CD1a immunostain but only dimly reactive for S100 protein. Ki67 highlighted a rare cell within the infiltrate. Staining for TdT was negative. The clinical and histologic findings were diagnostic of Langerhans cell histiocytosis.
