Description: symmetric purple discoloration of dependent skin with poor capillary refill
Comments: An adolescent with joint hypermobility and orthostatic intolerance demonstrates marked acrocyanosis. Note the two blanched finger prints on the top of the right foot showing poor capillary refill.

Description: cool purple tender toes
Comments: This 12 year old girl complained of exquisitely tender swollen right middle toes for 4 months. Pain did not worsen or improve with cold or heat exposure or administration of analgesics or peripheral vascular dilators. All four extremities were somewhat cool and cyanotic on examination. A thorough history, medical examination, and laboratory evaluation were normal.
