Comments: A 45-year-old man was evaluated for painful psoriasiform lesions of his fingers. On examination he was noted to have acral psoriasisform and erosive lesions on his ears and toes typical of Bazex syndrome (acrokeratosis neoplastica). Palpation of the left cervical area revealed a firm lymph node which is shown on this picture. Ear, nose, and throat examination demonstrated squamous cell carcinoma of the tonsil. Bazex syndrome is most often associated with squamous cell carcinoma of the upper aerodigestive tract.

Description: symmetric red to violaceous scaly atrophic plaques with associated nail dystrophy
Comments: This 73-year-old man was evaluated for a red scaly eruption on the hands, feet, and ears for 2 months. A biopsy showed changes suggestive of psoriasis. A computed tomographic scan of the thorax revealed a nodular mass in the left lung. Histology of the lung mass demonstrated a squamous cell carcinoma, and the skin findings were felt to be consistent with acrodermatosis paranepplastica of Basex.

Description: symmetric erythematous scaly patches with yellowing and scaling of the nails
Comments: A 70 year old man developed a psoriasiform dermatitis of the fingers, toes and ears (helix). He had a destructive nail dystrophy. A few weeks later the patient also developed cervical adenopathy. Otolaryngologic investigations revealed a layngeal squamous cell carcinoma.The Bazex syndrome or acrokeratosis paraneoplastica is a rare dermatosos associated with carcinomas of the upper aerodigestive tract with possible lymph node mestastases. The cutaneous lesions occuring in the syndrome are non metastatic skin involvment. The psoriasiform lesions often precede the diagnosis of the associated malignancy, or occur simultaneously. The reapparance of skin lesions may signal a recurrence of the tumor. Treatment consists in treatment of the underlying cancer. In some cases, acitretin or oral psoralen phototherapy could be prescribed.
