JAUNDICE /
PURPURA, TRAUMATIC
Category: Iatrogenic disorders, child abuse, /

Body Site: scalp / forehead
face
  Age: 2 days
Contributor: Christoph U. Lehmann, MD
Description: a large fluctuant red, purple swelling over the parietal and occipital areas; red bruises and abrasions over the forehead and cheeks; yellowish skin in unaffected areas
Comments: This infant was delivered via cesarean section after failed vaginal delivery with forceps and vacuum applied. She subsequently developed a large subgaleal hematoma resulting in significant blood loss requiring transfusion with packed red blood cells (PRBC) and fresh frozen plasma (FFP). The large majority of subgaleal bleeds in the newborn are associated with vacuum application. Note the mask to shield the eyes from phototherapy needed to treat the subsequent hyperbilirubinemia.
