Description: There is mild epidermal acanthosis with focal parakeratotic mounds. The mounds lift off the skin surface. There is also spongiosis and basilar vacuolopathy. Rare dyskeratotic cells are identified. Within the dermis, there is a mild perivascular and interstitial infiltrate, consisting of lymphocytes.
Comments: This is pretty typical histology of PR but a slightly atypical presentation. PR usually affects young adults and kids, and is truncal. The pityriasiform reaction pattern is a combination of spongiosis and interface. The lifting mounds are characteristic and correspond to the fine bran-like scale seen clinically.

Description: generalized but primarily truncal 0.5-3.0 cm round and annular hyperpigmented papules and plaques with dusky centers and gray borders with scale pointing centrally
Comments: This 12-year-old boy was noted to have widespread scaly round and annular plaques at a scoliosis screening visit. A skin biopsy showed dermatitic changes consistent with pityriasias rosea. This case was prepared by second year medical student at the University of Mississippi School of Medicine Claire E. Nettles.

Comments: A healthy 46-year-old woman developed a slightly pruritic eruption on the tops of the hands and feet and the arms and legs 2 weeks after an upper respiratory infection. The lesions improved with topical steroids and resolved 3 months later without scarring. She was on no meducations and a thorough medical evaluation revealed no underlying disorder.

Comments: This healthy 22-year-old man developed an asymptomatic geralized papulosquamous eruption with a predominance on the trunk. Note the typical Christmas tree pattern as the papules and plaques follow the lines of cleavage on the back.
