Description: symmetric violaceous annular papaules and plaques with minimal central scale
Comments: A 30-–year-old man infected with syphilis developed a chancre on the glans penis followed by swelling of the inguinal lymph nodes. The chancre resolved after 10 days, and symmetric round and annular 1 cm violaceous plaques with central scale appeared 5 months later on the palms and soles typical of secondary syphilis. Two months later he developed firm eroded plaqueson the inner aspect of the lips and gingiva. Treponema pallidum was identified from the oral lesions by polymerase chain reaction. A few months later his daughter was evaluated for a primary lesion on her lip.

Description: symmetric hyperpigmented scaly minimally elevated papules and plaques
Comments: This 13-year-old girl was evaluated for a rash of 3 weeks duration on her face, hands, and feet. She also had smooth topped hyperpigmented papules on the labia majora. Other than the skin rash her physical examination was normal. Screening for sexually transmitted diseases revealed posite HIV titers; a positive RPR with a titer of 1/128, a reactive Treponema pallidum microhemagglutination assay, a positive Human immunodeficiency test, and positive pregnancy test. This patient demonstrates the importance of screening for multiple sexually transmitted diseases in this setting.

Comments: This 15 year old boy complained of a rash on the palms and soles. The "copper penny" lesions characterized by reddish browm thick scaly papules about the size of a penny are typical of secondary syphilis. He denied a history of a chancre, but his serologic test for syphilis was positive and the rash cleared with penecillin therapy.
