Description: There is a cleft in a subepidermal plane. There is focal serum and few inflammatory cells within the cleft. The dermal papillae are rigid (festooning). A significant inflammatory infiltrate is not noted in the dermis.
Comments: This is a pretty typical presentation for porphyria cutanea tarda. The pathology is great for it, but identical pathology can be seen with other porphyrias and pseudoporphyria. Immunofluoresence studies (not performed in this case) can aid in distinction from other pauci-inflammatory subepidermal vesiculobullous dermatoses with deposition along the BMZ and around papillary dermal vessels. Porphyrin studies are recommended for more definitive diagnosis.

Description: This view of the cleft demonstrates degenerating keratinocytes and basement membrane eosinophilic material hanging from the roof, the so-called caterpillar bodies.

Comments: This 48 year old woman developed recurrent bullae, crusts and atrophic scars on the tops of her hands and feet. She also complained of increased skin fragility and worsening of the rash during the summer months. A skin biopsy showed changes typical of porphyria cutanea tarda. She did admit to drinking at least 3-4 beers a day. She took an occasional nonsteroidal anti-inflammatory drug for arthritis but no other medications, and there was no family history of blistering disorders.
