| Description: | There is prominent acantholysis in this biopsy, most evident in the superficial layers of the epidermis. Dyskeratosis is not present. It covers a fairly broad region. | ||||
| Comments: | This patient had a fairly typical clinical presentation for pemphigus foliaceus. The pathology is supportive with an acantholytic dermatosis. Pemphigus foliaceus, with antibodies to desmoglein 1, will have preferential involvement of the superficial layers, as in this case. Some cases are diagnostic dilemmas because they are too subtle. The upper layer may be stripped off, leaving something that resembles psoriasis. Direct (or indirect) immunofluoresence testing will confirm the diagnosis with intercellular (ICS) deposition. Rarely the immunofluorescence is negative and indirect IF or ELISA is recommended. | ||||
