Description: disseminated confluent red scaly patches and scattered crusted ulcers, plaques and tumors
Comments: This 45-year-old man had a 15 year history of chronic dermatitis treated symptomatically. On recent evaluation he had polymorphic lesions with eczematous dermatitis, ulcers, crusts, and tumors. A chest s-ray demonstrated hilar adenopathy, a complete blood count showed eosinophilia, and skin biopsies showed changes consistent with cutaneous T-cell lymphoma.

Description: Immunohistochemical stains demonstrate that the atypical mononuclear cell infiltrate is composed of CD3+ T cells that co-express CD4. An immunostain for CD30 demonstrates diffuse positivity within the atypical population. The atypical cells are negative for CD15, EMA and ALK. Expression of CD7 is largely retained. The surrounding inflammatory infiltrate shows scattered CD8+ T cells which are small and lack cytologic atypia.
Comments: These findings supported the diagnosis of a CD30+ lymphoproliferative disorder and in this patient anaplastic large cell lymphoma.
