A skin biopsy showed a subepidermal blister and direct immunofluorescence demonstrated IgG and C3 on the blister roof and floor related to the hemidesmosome antigens BP230 and BP180.

	*Description:*	Histologic sections of skin show separation of the epidermis from the dermis in a subepidermal plane. The blister cavity contains serum and inflammatory cells. The subjacent dermis appears to have an infiltrate as well as background solar elastosis. The flanking epidermis shows mild spongiosis.
	*Comments:*	This 70-year-old man developed multiple tense bullae, with an erythematous base, over his arms. His oral cavity was not involved. This is a typical presentation and histology for bullous pemphigoid. This case also demonstrated heavy linear IgG and C3 along the epidermal basement membrane zone (not shown). The antigens are related to the hemidesmosomes, BP230 and BP180.
