Although he denied trauma, he was an avid tennis player which may have resulted in repetitive trauma to the great toe nails. Note the shallow longitudinal grooves which were seen on all nails and represent a normal variant which increases with increasing age. | ||||

| Comments: | This 32-year-old man was noted to have neurosensory deafness and widespread red, scaly, scaly plaques at birth. Over the next decade he developed symmetric generalized warty plaques with most prominent involvement of the face and extremities. Hair was sparse at birth and decreased as the papules and plaques thickened on the scalp, axillae, and face. Nails were atrophic at birth, and yellowing and scaling with increased friability developed during the first year. Keratoderma of the palms and soles with a leather grain-like texture typical of KID syndrome was apparent by adolescence. Chronic conjunctivitis, photophobia, and keratitis were present as well as perioral furrowing. Dramatic hyperkeratosis of plaques on the scalp and feet required regular monitoring for squamous cell carcinoma. Note the scar from excision and grafting of a tumor on the top of the left foot. | ||||

| Comments: | A 45-year-old man was evaluated for painful psoriasiform lesions of his fingers. On examination he was noted to have acral psoriasisform and erosive lesions on his ears and toes typical of Bazex syndrome (acrokeratosis neoplastica). Palpation of the left cervical area revealed a firm lymph node which is shown on this picture. Ear, nose, and throat examination demonstrated squamous cell carcinoma of the tonsil. Bazex syndrome is most often associated with squamous cell carcinoma of the upper aerodigestive tract. | ||||

| Comments: | A 56-year old man was noted to have acrosclerosis and nail atrophy of fingers. He developed two squamous cell carcinomas on his left index that lead to an amputation. His father and one brother had similar findings. Sclerotylosis or Huriez Syndrome is a genodermatosis characterized by sclerodactyly, nail anomalies, and palmar-plantar keratoderma. Other associated findings include telangiectasias of the lip, flexural contractures of the fifth finger, and poikiloderma of the nasal skin. Patients with Huriez syndrome have a high risk of developing cutaneous squamous cell carcinomas as well as gastrointestinal tumors.
