Description: erythematous, annular, sclaly plaque with well demarcated border
Comments: A 10-year-old girl with an itchy rash on the chest was treated with a combination topical antifungal/steroid cream for 6 months. She experienced an improvement in symptoms, but the eruption recurred as soon as she discontinued therapy. A potassium hydroxide preparation showed branching septate hyphae, and she subsequently cleared with an oral antifungal medication.

Comments: This healthy 9-year-old girl had an itchy scaly patch on her mid frontal scalp a year ago which was treated with topical steroids. The patch improved but hair loss slowly progressed. She subsequently developed several scaly patches on her forehead and left temple which also improved but never cleared with topical steroids. On evaluation a potassium hydroxide preparation was florridly positive from hair bearing (endothrix)as well as non-hair bearing (septate branching hyphae) areas. She was started on oral ultramicrosized griseofulvin. Note the annular patch on the left temple.

Description: confluent hyperpigmented red patch with scattered pustules and diffuse fine scale
Comments: This adolescent boy was misdiagnosed with alopecia areata and treated with oral antibiotics and topical steroids. Although the erythema, pustules and itching subsided the scale and subtle erythema became confluent on the back of the neck and scalp. He eventually cleared when tinea capitis was diagnosed and he was treated with an oral antifungal medication.

Comments: This 28-year-old woman had itchy erythematous circinate plaques of the goin, both flanks, submammary region and the buttocks for 3 years. She was applying a cream consisting of betamethasone valerate 0.05 percent , gentamycin sulphate 0.1 percent , tolnaftate 1 percent , and idochlor hydroxyquin 1 percent with some improvement.
