Description: tender light brown 2-4 mm dermal papules
Comments: A 40-year-old man recently developed tender dermal nodules on the left side of his abdomen. Pain increased with palpation of the lesions. He was healthy, and there was no family history of similar nodules. A skin biopsy revealed a leiomyoma. The clinical differential diagnosis of a tender dermal nodule should include leiomyoma, eccrine spiradenoma, neuroma, dermatofibroma, angiolipoma, neurilemmoma, endometrioma, glomus tumor, and granular cell tumor.

Description: 3 red dermal nodules in a line
Comments: The first and largest tumor appeared by 3 years of age, and the other nodules developed over the last year. She complained of pain but they were not tender on palpation. Multiple piloleiomyomas are the most common variant of leiomyoma and usually present as firm, small, red or brown intradermal nodules arranged in a group or line. They may be tender and give rise to spontaneous attacks of pain. A biopsy was taken from the middle of the center nodule.

Description: multiple symmetric 2-4 mm red follicular papules/nodules
Comments: This young woman developed increasing numbers of red follicular papules on her legs, thighs, and a few in her axillae. The papules were painful with palpation but generally asymptomatic. Her sister had similar papules on her upper back. Multiple piloleiomyomas are the most common type of leiomyoma presenting as linear and follicular firm red to brown intradermal nodules. Many areas may be affected including the legs, arms, axillae, neck, and torso. The nodules are usually tender especially with lateral pressure and may give rise to episodic painful attacks.

Description: The muscle fibers composing the tumor are generally straight with centrally located, thin, long, blunt edged, eel like nuclei. The fibers stain pink with hematoxylin-eosin similar to the staining of the collagen fibers in the tumor.
Comments: The nodules are composed of interlacing bundles of smooth muscle fibers with varying amounts of collagen bundles intermingled.
