Description: widespread clusters of 2-3 mm vesicles and crusted vesicles on a red base
Comments: This 8-month-old boy developed high fever, increasing fussiness, and a widespread blistering eruption on his face primarily, with a few lesion scattered on his hands upper chest, neck, and inside his mouth. A viral culture was positive for Herpes simplex virus type 1. His mother was noted to have a healing cold sore on her lower lip. He was admitted to the hospital for intravenous hydration, and the eruption improved within 48 hours with parenteral acyclovir. He was discharged on oral valacyclovir to complete a 10 day course of therarpy.

Description: The Quick Tzanck Test of the vesicle revealed 3 epidermal sheets. In their dermal side, destruction and replacement of the hair follicular epithelial cells by many balloon cells together with giant cells were observed .

Description: At higher magnification, groups of balloon cells and giant cells were observed around the hair shaft. Eosinophlic inclusion body in the nucleus were also observed. The patient was treated with 5 days of valacyclovir, 12 days of anti-allergic agents and class$B|u(Blocal steroid. There was no recurrence for 6 months. Not only the content and base of the vesicle were examined, but also the vesicle itself were included routinely. In this case, the epidermal sheets of the vesicles were observed from the corneum side. The viral cytopathic changes in the follicular epithelium were also clearly observed.
Comments: This 43-year-old woman with eczema over her eyelids for a year treated with topical steroids for 6 months developed vesicles and erosions 4 days ago. A Quick Tzanck test was supportive of Herpes simplex virus infection.
