| Description: | Panel-A shows the clinical findings. Panel-B,C are photomicrographs which show hyperkeratostic epidermis with acanthosis and focal thinning overlying dilated lymphatics. Deeper dermis shows dilated lymphatic channels. The channels are lined by endotlelium and contain proteinaceous fluid. Panel-D shows fluid in lymphatics with mature lymphocytes and no red blood cells. These features are typical of lymphangioma circumscriptum. Lymphangioma circumscriptum primarily affects the subcutaneous tissue in the form of cystic dilatation of lymphatic channels without systemic lymphatic communication. The skin lesions are probably secondary to the increased intraluminal pressure.Hence multiple skin vesicles and lymphorrhea can be clinically appreciated. The main treatment modality is surgical excision. Recurrences are not unusual and require re-excision. | ||||
| Comments: | This 5-year-old girl had multiple clustered thick walled copper vesicles on the right axillaryb fold for 4 years. It was excised a year ago but promptly recurred within the surgical scar. Fine needle aspirate cytology yielded turbic yellow fluid with mature lymphocytes in a proteinaceous background. | ||||
