	*Description:*	At higher magnification, groups of balloon cells and giant cells were observed around the hair shaft. Eosinophlic inclusion body in the nucleus were also observed. The patient was treated with 5 days of valacyclovir, 12 days of anti-allergic agents and classⅴlocal steroid. There was no recurrence for 6 months. Not only the content and base of the vesicle were examined, but also the vesicle itself were included routinely. In this case, the epidermal sheets of the vesicles were observed from the corneum side. The viral cytopathic changes in the follicular epithelium were also clearly observed.

	*Comments:*	This 64- year-old man suffered from recurrent Herpes simplex labialis for 10 years. Vesicles over a red plaque accompanied by burning and itching developed on the right cheek 5 days ago. The Quick Tzanck test of material obained from the vesicles revealed balloon cells with thick nuclear and cytoplasmic membranes fusing to form multinucleate giant cells. The eruption cleared in 10 days with topical antibiotic ointment, topical steroid, and 5 days of oral valacyclovir. The Quick Tzanck Test, which incorporates a modified Giemas solution, was devised by Dr. Sino as a rapid screen for the diagnosis of Herpes simplex virus infection. Staining for cytology takes only 2 minutes.

A skin biopsy showed granulomatous inflammation with changes typical of Herpes simplex virus infection, and the viral culture was positive for Herpes simplex virus type I.
