This 3-day-old infant, who was delivered 3 weeks premature, developed discrete and confluent red papules and plaques the previous night which became studded with pustules. The lesions quickly evolved and disappeared within 48 hours. Although it was unlcear why the lesions on the back became confluent, the baby spent the first 24 hours of life on a warmer and the findings might represent a koebner phenomenon triggered by warming of the back while erythema toxicum was evolving. Cultures from the skin and blood for bacteria and herpes simplex virus were negative.
