This 75-year-old woman developed an ulcerated plaque in the right cubital fosssa 5 months earlier. She had a history of receiving an intravenous drug at this site which caused necrosis and ulceration followed by scarring many years ago. The lesion bled even with gentle manipulation. Histology showed an infiltrating well differentiated squamous cell carcinoma. Six months after excision she returned with metastases to the axilla.
