This 55 year old man with pemphigus vulgaris treated with high dose oral prednisone for 1 year developed tender ulcerated violaceous nodules and plaques on his thighs a month earlier. He subsequently developed fevers, chills, night sweats, and headaches. A skin biopsy showed organisms in the deep dermis and fat consistent with cryptococcus. The organism was also found in spinal fluid. His symptoms and rash had improved since he was started on parenteral amphotericin.
