This febrile 4 year old boy developed a painful expanding plaque on the back of his left hand. He was aplastic with an absolute neutrophil count of 0 after completing an induction course of chemotherapy for leukemia. The plaque began at the site of an infiltrated intravenous catheter that was occluded with a semipermeable dressing. A biopsy revealed necrotic epidermis and dermis with fungal organisms invading vascular structures. The plaque was subsequently excised and grafted with good healing and no evidence of residual fungal infection.
