This 10-year-old girl had a 2 month history of intermittent fever and generalized erythematous macules. She had bilateral thigh pain, weight loss, and lethargy. Examination of her hands revealed subtle red papules on the knuckles of both hands. Magnetic resonance imaging indicated calification of the thigh muscles. The case was prepared by senior medical student at the University of Mississippi Katie L. Maxwell.
