This 84-year-old man with a history of myelodysplasia developed a progressive painful red patch with a central crust on his right arm several days after abrading his arm when he fell on the carpet. Initially he improved slowly on parenteral cefazolin followed by oral cephalexin. Blood cultures were negative and he remained clinically well.
