This condition is commonly associated with vascular insufficiency, especially stasis changes. A frequent clinical presentation is an "apple-core" or "inverted champagne bottle" appearance to the lower legs. The differential diagnosis would include other forms of mixed septal and lobular panniculitis, including factitial etiologies.

Description

At higher magnification, adipocytes in the subcutaneous fat have coalesced into microcysts with an amorphous eosinophilic material thickening their walls.A mild lymphocytic inflammatory infiltrate is present in the septae and reticular dermis. A corrugated or crenulated appearance to the adipocyte walls is seen.
