A healthy newborn developed scattered 3-5 mm irregularly shaped round to lancet shaped vesicles on a narrow red base at 5 days of age. Her liver enzymes were dramatically elevated, she began to feed poorly, and she developed a fever. After multiple cultures were obtained she was treated with parenteral acyclovir, and her clinical and laboratory parameters normalized within several days as the vesicles began to heal.
