This 15-year-old boy developed clusters of asymptomatic purpuric macules over the right thigh after five days of fever, pharingitis, and diffuse lymphadenopathy. Serologic studies revealed acute Epstein-Bar virus infection, and thrombocytopenia was noted on a complete blood count. A more extensive evaluation revealed antiplatelet antibodies and autoimmune thrombocytopenic purpura (ATP) from EBV infection was diagnosed. In children ATP usually occurs abruptly following a viral or bacterial infection, and spontaneous recovery develops within days to weeks. In EBV and Human immunodeficiency virus associated-ATP the course may be more protracted.
