Comments

This adolescent boy developed a keratoderma with dramatic scaling on his palms and soles particularly over weight bearing surfaces. Topical keratolytics produced little improvement, and a biopsy showed dermatitic changes. He cleared with oral prednisone and remained clear on topical steroids and calcipitriol. An acquired keratoderma can occur in familial keratodermas, psoriasis, atopic dermatitis and a number of other disorders. Unfortunately, when lesions are restricted to the palms and soles and histologic findings are not specific, it may not be possible to make a definitive diagnosis.
