A recent immigrant from South America gave a 5 year history of a rash on his right arm and shoulder. The plaques were only minimally pruritic. Skin biopsy demonstrated granulomatous inflammation and cultures grew Mycobacterium tuberculosis. These chronic lesions are typical of lupus vulgaris.
