A soft compressible mass appeared several months earlier on the left side of the neck in this healthy adolescent. Branchial cleft cysts occur in the upper lateral neck along the anterior border of the sternocleidomastoid muscle. They are most commonly diagnosed between 20 and 40 years of age and probably arise from remnants of the branchial clefts with the majority associated with the second branchial arch. The lesion was surgically excised.
