A recent immigrant from South America gave a 5 year history of a rash on his right arm and shoulder. The plaques were only minimally pruritic. Skin biopsy demonstrated granulomatous inflammation and cultures grew Mycobacterium tuberculosis. These lesions are typical of lupus vulgaris.

large fibrotic hypopigmented plaques with central atrophy, scattered crusts and peripheral hyperpigmentation; firm mass on shoulder with overlying hyper and hypopigmentation
