This 35-year-old Human Immunodeficiency Virus positive woman was evaluated for neck swelling of 2 months duration associated with fever and night sweats. The area was violaceous but asymptomatic. Needle aspirate yielded purulent material, and the Ziehl-Neelsen stain for acid fast bacilli was positive.
