A 14 year old boy with widepsread resistant psoriasis was evaluated for treatment options. He had faiiled topical steroids, calcipotriol, lubricants, coal tar, and tazarotene, phototherapy, and oral cyclosporine. Note the sparing between his knuckles. When scale was accidentally scraped from his hands he developed bleeding characteristic of the Auspitz sign seen in psoriasis.
