This 40-year-old man, who had acanthosis nigricans for the last several years, developed a chronic folliculitis and perifolliculitis of the occipital scalp and posterior neck. Pustules healed with firm violaceous follicular papules and nodules over three years. He developed similar lesions on his back and flanks below the axillae. He was treated with topical tretinoin and oral antibiotics.
