This 48-year-old man with a history of gout (recurrent arthritis for the last 8 years) presented with a 10 month history of increasing numbers of gradually enlarging papules of the auricle. The lesions were asymptomatic and the patient denied bleeding or drainage. On physical exam there were several pale, nontender nodules measuring 2 to 8 mm in diameter over both auricles, primarily the helix and antihelix. These were very superficial, well-circumscribed, and with no surrounding erythema or ulceration. The ear is one of the most common sites of tophus formation and is the most common location in the head and neck.
