An 82-year-old woman developed itchy tense bullae on a red base over her arms, trunk, and legs over 2 months. Mucous membranes were spared. A skin biopsy showed a subepidermal blister and direct immunofluorescence demonstrated IgG and C3 on the blister roof and floor related to the hemidesmosome antigens BP230 and BP180.

Description

There is a subepidermal blister containing a mixed infiltrate of lymphocytes, eosinophils, and neutrophils. A lymphocytic infiltrate is seen in the dermis.
