This 45-year-old woman with diabetes developed a painful red plaque on her left calf. The area enlarged and blistered at the inferior advancing border. She had no history of trauma or the application of topical medications. A Gram stain showed gram positive cocci. She had a complete but slow response to a parenteral antibiotic(cefotaxim) which was administered in the hospital. After initial improvement she finished with oral antibiotics at home.
