This patient developed palpable purpura on his lower extremities. The clinican suspected Churg-Strauss as he has a long history of asthma and peripheral eosinophilia. The histology in this case is pretty typical. There is an acute vasculitis and eosinophilia. The third finding, not evident in this case, is interstitial granulomas, giving this its other name of allergic granulomatosis. In this case, there is vasculitis involving both small and medium sized vessels, not uncommon with this entity.

Description

Within the dermis, there is a patterned inflammatory infiltrate. It appears to follow vessels and extend deep. Both the superficial and deep vascular plexi are affected.
