A 10 year old boy with atopic dematitis had the most severe disease on the tops of his hands. He improved with moisturizers and topical steroids as well as avoidance of irritants. Note the hyperpigmentation of acanthosis nigricans over his knuckles and proximal interphalyngeal joints.
