Comments

This 54 year old man presented with numerous small 1-2 mm scaly papules over his chest and back. This is a typical presentation for transient acantholytic dermatosis (Grover disease). The pathology can be variable, with pemphigus-like, Hailey-Hailey-like, Darier-like, and spongiotic forms. Without dyskeratosis, this one could be placed in the Hailey-Hailey-like or pemphigus-like categories. The distinction is based on the clinical findings and that the histologic changes in Grover disease are often limited to 2-3 retia, not broad lesions.

Description

The individual lesions span several retia. There is prominent acantholysis without dyskeratosis.
