This 4-year-old boy developed a 1 cm scaly plaque on the occiput with ipsilateral occipital adenopathy. A fungal culture of scale from the scalp was positive for Trichophyton tonsurans, and oral griseofulvin was prescribed. He did not take his medication and returned several months later with multiple tender boggy crusted areas on the scalp. This photograph shows follow-up one week after initiating therapy with oral cephalexin and griseofulvin and topical selenium sulfide shampoo.
