This 56-year-old African American developed progressive swelling of the tops of herl great toes over the past 6 years. These lesions are non tender but slightly pruritic. The patient’s past medical history was significant for thyroidectomy 12 years ago while in Ghana. The patient notes that approximately 4 years ago, her sister-in-law mentioned that her fingers looked strange.
